Information and forms for your child’s school to help educate them when your child has a medical concern (asthma, diabetes, seizures) or life-threatening food or bee allergy. Forms include a Quick Food Allergy Action Plan, Epipen® and Twinject™ directions, Responsibilities of the School, and a Food Allergy Assistance Plan.
